Scaling and planing is a type of deep cleaning that aims to help the gum tissue to heal and periodontal pockets to shrink. In the first part, scaling, the dentist removes all the plaque and tartar (hardened plaque) above and below the gumline using hand instruments. The dentist then planes the tooth roots by smoothing them out to help the gums reattach to the teeth. Planing helps keep bacteria from collecting in the form of plaque on the tooth surface. The procedure could take more than one visit to complete and may require anesthetic. Following the procedure, the dentist may recommend medications to help control infection and pain, or to aid in healing.
If the periodontal pockets do not heal after the scaling and planing procedure, surgery may be necessary. During periodontal surgery, inflamed tissues are removed to prevent damage to the bone. If left to enlarge, the pockets provide a place for bacteria to live and attack the bone and tissue. Periodontal surgery can also include bone surgery, including bone grafts, and soft-tissue grafts.
