Dental scaling is a type of cleaning that uses a special tool to reach plaque and tartar below the gum line. The roots of the teeth are often planed following the scaling, which is good for gum health. Tooth scaling can improve and prevent a wide range of potential gum and teeth issues. Root planing is like tooth scaling except it takes place on the roots of your teeth that sit below your gum line. During root planing, your doctor uses a tool to gently push aside your gum tissue and expose the surface of the roots. Scaling and planing is a type of deep cleaning that aims to help the gum tissue to heal and periodontal pockets to shrink. In the first part, scaling, the dentist removes all the plaque and tartar (hardened plaque) above and below the gumline using hand instruments. The dentist then planes the tooth roots by smoothing them out to help the gums reattach to the teeth. Planing helps keep bacteria from collecting in the form of plaque on the tooth surface. The procedure could take more than one visit to complete and may require anesthetic. Following the procedure, the dentist may recommend medications to help control infection and pain, or to aid in healing.

If the periodontal pockets do not heal after the scaling and planing procedure, surgery may be necessary. During periodontal surgery, inflamed tissues are removed to prevent damage to the bone. If left to enlarge, the pockets provide a place for bacteria to live and attack the bone and tissue. Periodontal surgery can also include bone surgery, including bone grafts, and soft-tissue grafts.
